Your Form 1095-C for the 2025 tax year is now ready for review and download. This form will no longer be provided by mail. This document provides important information about your health insurance coverage that may be needed for your tax filings.
To access your digital copy, please log into Workday, navigate to the Benefits app (or search “My ACA Forms” in the search bar), and select View/Print for the 2025 statement (refer to Accessing your 1095-C Forms for step-by-step instructions). If you do not have access to Workday or need assistance accessing your form, please contact the HR Service Center.
